Community Relationship Manager - Southern Mornington Peninsula Uniting Church
The Southern Mornington Peninsula has unique population demographics. It is home to twice the state’s average of people aged sixty and over, some of whom face the challenge of caring for a disabled family member. The National Disability Insurance Scheme (NDIS) provides support for the disabled person, but support for extended family members and carers is limited.
Seeking to address this deficit, and to respond to ever-changing community need, the Southern Mornington Peninsula Uniting Church (SMPUC) established a Carer Support and Outreach Service. This service focuses on two main areas: the psychosocial support and nurturing of community relationships and the provision of Specialist Disability Accommodation (SDA) for up to eight NDIS SDA eligible Participants within a Platinum level, High Physical Support block of 4 fully self-contained, independent apartments.
Toorak Uniting Church Outreach provides financial support to SMPUC for them to employ a Community Relationship Manager (CRM). Samantha, the Community Relationship Manager, works with local families in assisting them to prepare for the SDA application process, provides psychosocial support to carers and during the Covid-19 crisis has extended the role to include providing training for congregation members to access online meeting software such as Zoom which has contributed to ongoing community connection during times of social isolation.
With the recent employment of an SDA Provider who will manage the day to day operational aspect of running an SDA property, the Community Relationship Manager will ensure the smooth transition to tenancy of all successful applicants and their families and will act as the “on the ground” liaison between tenants, SMP Church Council, UCA Vic/Tas and the SDA Provider.
The Community Relationship Manager is a co-opted Committee member of Church Council and is accountable to the Chairperson, Jill Fearon.
